Approve three contract amendments funded by Public Health: a first contract amendment with Persimmony International, Inc. from $236,748 to $318,460 effective March 28, 2017 through the current contract period of June 30, 2018 to expand the number of users by 16 for the electronic health record to include the Communicable Disease Program; a second contract amendment with SIMI Group from $255,000 to $385,500 effective March 28, 2017 through the current contract period of June 30, 2017 to develop an electronic tool in the community health care clinics to track and assess social determinants of health and other health data; and a second contract amendment with Full Court Press Communications, Inc. from $271,751 to $298,251 effective March 7, 2017 through the current contract period of June 30, 2017 to expand media outreach and advertising to promote chronic disease prevention and early intervention activities; and Authorize the County Administrator to execute the amendments and any subsequent amendments that remain within budgeted appropriations
