DEPARTMENTAL RECOMMENDATION:

 

The Department of Health and Social Services (H&SS) recommends that the Board of Supervisors approve a contract with JJM Integrated Health, Advanced Nursing Inc. for $120,000 to provide Behavioral Health transcranial magnetic stimulation services for the period of May 1, 2025 through June 30, 2026; and Delegate authority to the County Administrator to execute the contract and any subsequent amendments, with County Counsel concurrence, up to an aggregate of $74,999.

 

SUMMARY/DISCUSSION:

 

Solano County Behavioral Health continues to expand its array of services to meet clients’ general and unique service needs as required by the Department of Health Care Services (DHCS) Medi-Cal Specialty Mental Health Services (SMHS) contract. The customary treatment approach considered to be industry practice for mental health care includes a combination of medication treatment, psychosocial therapies/rehabilitation activities, and case management. Certain individuals do not respond to commonly used medications and therapies, and their conditions are therefore considered “treatment resistant.”

 

Solano County Behavioral Health has worked to find alternative care methodologies to provide efficacious care for plan members whose symptoms have not responded to traditional treatment methods. An alternative for those with medication treatment resistance is Transcranial Magnetic Stimulation (TMS), which involves weeks of ongoing treatment in specialty clinics with specialized equipment. Research has shown that TMS can demonstrate successful results. Until now, Solano County has not had access to TMS as a treatment alternative. JJM Integrated Health, Advanced Nursing Inc. provides TMS services and is located within Solano County.

 

Additionally, JJM Integrated Health, Advanced Nursing Inc. will be submitting a Medi-Cal provider enrollment application to DHCS as California’s Medi-Cal program now covers TMS for eligible clients. JJM’s enrollment in Medi-Cal as a certified agency will allow Solano County to draw down Medi-Cal funding from DHCS to help cover the cost of these services for a small subset of clients annually.

 

FINANCIAL IMPACT:

 

This contract is funded with 2011 Realignment, offset with federal financial participation generated through Medi-Cal claiming. Appropriations for this agreement are available in the FY2024/25 Working Budget and will be included in the department’s FY2025/26 Supplemental Budget request. There is no additional financial impact on the County General Fund.

 

ALTERNATIVES:

 

The Board of Supervisors may choose not to adopt this contract. However, this is not recommended as TMS is an evidence-based procedure that offers an alternative treatment for certain individuals who have been unsuccessful achieving symptom reduction utilizing other forms of treatment.
