DEPARTMENTAL RECOMMENDATION:

 

The Department of Health & Social Services (H&SS) recommends that the Board approve a renewal contract with Medical Hill Rehabilitation Center for a total of $126,000 over two years to provide sub-acute inpatient residential psychiatric medical services for the period beginning July 1, 2017 through June 30, 2019; and Authorize the County Administrator to execute the agreement and, with concurrence from County Counsel, any amendments that are within budgeted appropriations up to 20% of the total contract amount, not to exceed $50,000.

 

SUMMARY:

 

Medical Hill Rehabilitation Center, LLC provides treatment to the complex, disabled adult population with neuro-behavioral impairments who require a more structured treatment environment than can be provided in community treatment settings. The population served has very severe needs such as brain injury in addition to psychiatric needs and requires extensive monitoring and assistance. This proposed total contract for $126,000 over two years for the period beginning July 1, 2017 through June 30, 2019 will ensure continued psychiatric coverage to meet the service needs for consumers currently in that facility and other consumers who may require this specialized level of care.  During FY2016/17, approximately $55,000 was expended on the current Medical Hill Rehabilitation Center, LLC contract.

 

FINANCIAL IMPACT:

 

The contract is funded with approximately 12% Federal Short Doyle, 76% 1991 Realignment, and 12% County General Fund. Appropriation is included in the Department’s FY2017/18 Recommended Budget. There is no additional impact to the County General Fund.

 

DISCUSSION:

 

Medical Hill Rehabilitation Center, LLC provides sub-acute mental health inpatient rehabilitation and stabilization services in a secured setting for seriously and persistently mentally ill individuals who also suffer with neuro-behavioral conditions. The facility provides a comprehensive, therapeutic supervised program focusing on behavioral expectations, rehabilitation, medication, and social skill training.

 

Program specific activities include individualized medication education; support and services in a collaborative manner with the goal of enabling clients to monitor their own medication; coordination with the clients’ primary care physicians; appropriate staffing 24 hours a day, 7 days a week; educational and vocational opportunities; and individualized treatment plans that focus on measurable goals and specific activities to be provided by board and care to assist clients in reaching goals in the plan.

 

Mental health services are strength-based, person-centered, safe, effective, timely, and equitable with an emphasis on promoting wellness and recovery. A written performance measure report will be provided on a quarterly basis to monitor clients’ progress. A quality improvement plan and quality assurance process will also be developed and maintained to include: internal quality improvement work plan with goals around timeliness, quality and outcomes, and monitoring the safety and effectiveness of medication practices. Overall, clients benefit from an interdisciplinary clinical team to provide a comprehensive approach to patients’ needs. 

 

ALTERNATIVES:

 

The Board may choose not to approve the contract with Medical Hill Rehabilitation Center, LLC.  This is not recommended as the County is required to have psychiatric services for clients served in the County mental health system. 

 

OTHER AGENCY INVOLVEMENT:

 

County Counsel has reviewed the contract for legal sufficiency.
