DEPARTMENTAL RECOMMENDATION:
The Department of Health and Social Services recommends that the Board of Supervisors accept a California Department of Public Health Tuberculosis Control Branch allocation award of $121,972 to support the California Tuberculosis Controllers Association (CTCA) tuberculosis prevention and control activities; approve a $101,709 contract with San Francisco Public Health Foundation to support the project goals of the CTCA for the period of January 1, 2025 through June 30, 2025; delegate authority to the County Administrator to accept the award and other related documents necessary to receive the award; and delegate authority to the County Administrator to execute the contract and any subsequent amendments, with County Counsel concurrence, up to an aggregate of $74,999.
SUMMARY:
The California Department of Public Health (CDPH) Tuberculosis Control Branch (TBCB) awards local assistance funds to support tuberculosis (TB) activities to prevent, control and eventually eliminate TB in California. These funds were awarded to Solano County Public Health with the understanding that staff will work with the TBCB in carrying out CDPH-funded TB control program efforts. The contract with San Francisco Public Health Foundation will support the project goals of the California Tuberculosis Controllers Association and will provide program administration, program management and administrative community capacity building services. Additionally, San Francisco Public Health Foundation will convene local county TB controllers in two statewide meetings to determine and highlight best practices, develop statewide guidelines, and measure the implementation and effectiveness of TB-related policies.
FINANCIAL IMPACT:
CDPH, TBCB awarded Solano County Public Health $121,972 to support the contract with San Francisco Public Health Foundation. This award was anticipated, including the contract with San Francisco Public Health Foundation, and included in the Department’s FY2024/25 Working Budget. The cost of preparing this agenda item is nominal and is absorbed by the Department’s FY2024/25 Working Budget. There is no additional impact on the County General Fund.
DISCUSSION:
TB remains one of the world’s deadliest infectious killers. Fortunately, TB disease is preventable and curable with medication. In California, local health jurisdictions work tirelessly to quickly identify and treat residents who have TB, identify those who may have been exposed to it, and provide them with appropriate treatment. These activities reduce the spread of disease in California communities.
Per the California Department of Public Health Tuberculosis Control Branch, in 2023, 2,113 new TB cases were reported, a substantial increase (15%) compared with 1,842 in 2022. California’s annual TB incidence was 5.4 cases per 100,000 persons. This is nearly double the national TB incidence rate of 2.9 cases per 100,000 persons as reported by the Centers for Disease Control and Prevention. The rate of TB is even higher in Solano County, an incidence rate of 5.7 per 100,000 persons (26 cases in 2023). Ending TB will require a dual approach of (1) maintaining and strengthening TB control priorities, and (2) increasing efforts to identify and treat latent TB infection, especially in populations at increased risk for TB.
CDPH and Local Health Jurisdictions formed the California Tuberculosis Controllers Association (CTCA) in 1984, which is committed to creating the opportunity for optimal health for all Californians by working to eliminate barriers to TB diagnosis, treatment and prevention, and prioritizing communities disproportionately impacted by TB while working to eliminate the threat of TB for all Californians.
ALTERNATIVES:
The Board may choose not to accept this award and not to approve the contract with San Francisco Public Health Foundation to support tuberculosis prevention and control activities. This is not recommended because Solano County and its neighboring jurisdictions benefit from the guidelines and services CTCA provides.
OTHER AGENCY INVOLVEMENT:
Solano County Health and Social Services worked with the California Department of Public Health, the San Francisco Public Health Foundation, and the California Tuberculosis Controllers Association members to develop the contract with the San Francisco Public Health Foundation.
