DEPARTMENTAL RECOMMENDATION:

 

The Solano County Department of Health and Social Services recommends that the Board of Supervisors accept three revenue allocations for a net amount of $830,113 as follows:  $366,314 from the U.S. Health Resources and Services Administration, $77,101 from the U.S. Department of Health and Human Services, and $386,698 from the U.S. Department of Health and Human Services; and approve an Appropriation Transfer Request (ATR) for $830,113 to recognize the unanticipated revenues and budget appropriations for clinical and non-clinical equipment to support Health and Social Services, Family Health Services operational needs to prevent and respond to COVID-19 for the current period through June 30, 2022 (4/5 vote required).

 

SUMMARY/DISCUSSION:

 

The Solano County Department of Health and Social Services (H&SS) has been awarded three revenue allocations totaling $830,113 to support Health and Social Services, Family Health Services most pressing needs to prevent and respond to the coronavirus disease 2019 (COVID-19). 

 

The first allocation of $366,314 represents Year 1 of the U.S. Health Resources and Services Administration (HRSA) Bureau of Primary Health Care (BPHC) American Rescue Plan - Health Center Construction and Capital Improvements (ARP Capital) funding. The total ARP Capital funding of $749,678 is to be used over three years (Year 1 = $366,314, Year 2 = $315,837, and Year 3 = $67,527) to support the County’s Family Health Services (FHS) clinics through enhancing and expanding the existing approved medical and dental services and service sites' infrastructure in response to COVID-19 and other emerging primary health care infrastructure needs.

 

FHS will use the ARP Capital Year 1 funding of $366,314 to purchase clinical and non-clinical movable equipment items:

 

                     Equipping and/or replacing three FHS medical clinic service sites with reclining phlebotomy chairs

                     Equipping and/or replacing exam tabletops and power chairs and eye test units at all

four FHS medical clinic service sites

                     Equipping and/or replacing three FHS pediatric medical clinic service sites' baby scales

                     Replacing vital signs monitors and associated mobile stands at all FHS medical and dental clinic service sites

                     Equipping and/or replacing laptops, desktops, scanners, and the associated accessories

 

All purchases will be used to facilitate enhanced in-person and telehealth adult and pediatric primary and dental care.

 

The second allocation of $77,101, called the ‘ARP Rural Plan’, was awarded by the U.S. Department of Health and Human Services (HHS) to rural healthcare service providers impacted by the COVID-19 pandemic.  HHS awarded $77,101 to Solano County H&SS for its share as a Federally Qualified Health Center (FQHC) to support healthcare-related expenses and lost revenue attributable to COVID-19.  The ARP Rural Plan funding will be used to cover some of the revenue losses in the County’s FHS clinics. 

 

The third allocation of $386,698 was awarded by HHS to providers who have experienced changes in operating revenues and expenses because the COVID-19 pandemic.  These funds resulted from the passage of the federal Coronavirus Preparedness and Response Supplemental Appropriations Act of 2020 (Supplemental Act), signed by former President Trump on March 6, 2020. This funding opportunity is called the Corona Response and Relief Supplemental Appropriation - Provider Relief Fund (PRF).  The PRF funding of $386,698 will be used to cover some of the revenue losses in the County’s FHS clinics. 

 

FINANCIAL IMPACT:

 

Solano County Family Health Services will receive and administer the total federal funds of $830,113 from HRSA and HHS. The ATR reflects a reduction of $581,008 in the use of 1991 Public Health Realignment and a $249,105 increase in appropriations funded by these federal revenues with no County General Fund impact.

 

The costs associated with preparing the agenda item are nominal and absorbed by the Department’s FY2021/22 Adopted Budget.

 

ALTERNATIVES:

 

The Board of Supervisors could choose not to authorize acceptance of these funds and not to approve the ATR. This is not recommended, as the proceeds will fund acquisition of critical equipment and support personnel.
