DEPARTMENTAL RECOMMENDATION:
The Department of Health and Social Services recommends that the Board of Supervisors:
1. Delegate authority to the County Administrator or designee to negotiate a seventh amendment to the revenue agreement with Medic Ambulance Service, Inc., with County Counsel concurrence, to extend the contract term through February 1, 2027, with an additional two month franchise fee of $119,834; and
2. Delegate authority to the County Administrator or designee, to execute the amendment and any subsequent amendments, with County Counsel concurrence, to ensure continued delivery of ambulance services.
SUMMARY/DISCUSSION:
On December 2, 2025, the Solano County Board of Supervisors adopted an ordinance amending Solano County Code Chapter 7.1, designating the Solano County Health and Social Services Department (H&SS), Public Health Division, as the Local Emergency Medical Services Agency (LEMSA) in place of the Solano Emergency Medical Services Cooperative (SEMSC). A LEMSA is a governmental entity designated by a county to plan, implement, and oversee the local emergency medical services system. This designation became effective January 2, 2026.
Medic Ambulance Service, Inc. (Medic) is the current provider of advanced life support ambulance services within the exclusive operating area located in Solano County. H&SS is in negotiations for a seventh amendment to the agreement with Medic to extend the contract term through February 1, 2027 to allow uninterrupted service delivery during contract negotiations with the City of Fairfield which are underway as a result of a procurement process. Negotiations with Medic on the terms of the seventh amendment are ongoing, with discussions focused on the proposed mileage rates and associated fee structure.
SEMSC, in its capacity as the previous LEMSA, executed the original agreement on April 8, 2010, for a contract term of May 1, 2010, through May 1, 2015. On October 3, 2014, a first amendment was executed to extend the term through May 1, 2020. A second amendment was implemented on October 9, 2014, to provide that, if certain conditions were met, the contract would be extended for an additional five years through May 1, 2025, and to authorize annual adjustments to Medic’s rates. On June 13, 2019, a third amendment was executed to exercise the five-year extension through May 1, 2025. A fourth amendment was executed on April 1, 2024, to increase the franchise fee through May 1, 2025. On May 1, 2025, the agreement was amended for a fifth time to extend the term through May 1, 2026, and to increase the franchise fee. On March 10, 2026, a sixth amendment was approved by the Solano County Board of Supervisors to acknowledge the change in the LEMSA agency and extend the term through December 1, 2026. The current proposed seventh amendment would extend the term through February 1, 2027.
FINANCIAL IMPACT:
The proposed amendment extends the revenue agreement with Medic from December 1, 2026, through February 1, 2027. Under this extension, the annual franchise fee will remain unchanged at $719,000. This revenue will be prorated and billed monthly, resulting in a monthly franchise fee of $59,917 and a total franchise fee payable for the two-month extension of $119,834.
ALTERNATIVES:
The Board of Supervisors may choose not to approve the delegated authority to negotiate and execute this seventh amendment. This is not recommended, as it may adversely affect essential advanced life support ambulance services within the exclusive operating area.
OTHER AGENCY INVOLVEMENT:
None.
