DEPARTMENTAL RECOMMENDATION:
The Department of Health and Social Services recommends that the Board of Supervisors:
1. Approve a sixth amendment to a revenue agreement with Medic Ambulance Service, Inc., to extend the term of the contract from the current period through December 1, 2026, to increase the annual franchise fee by $50,000 for a total annual franchise fee of $719,000, and to change the contracting authority from the Solano Emergency Medical Services Cooperative to the County of Solano; and
2. Delegate authority to the County Administrator to execute the amendment and any subsequent amendments, with County Counsel concurrence, up to $74,999 per fiscal year.
SUMMARY/DISCUSSION:
On December 2, 2025, the Solano County Board of Supervisors adopted an ordinance amending Solano County Code Chapter 7.1 designating the Solano County Health and Social Services Department (H&SS), Public Health Division, as the Local Emergency Medical Services Agency (LEMSA) in place of the Solano Emergency Medical Services Cooperative (SEMSC). A LEMSA is a governmental entity designated by a county to plan, implement, and oversee the local emergency medical services system. This designation became effective January 2, 2026.
The Department of Health and Social Services requests approval of a sixth amendment to the agreement with Medic Ambulance Service, Inc., to change the contracting authority from SEMSC to Solano County reflecting the Solano County Board of Supervisors as the new governing body of the LEMSA, to extend the term of the contract through December 1, 2026, and to increase the annual franchise fee by $50,000 beginning May 1, 2026.
Medic Ambulance Service, Inc. (Medic) is the sole provider of advanced life support ambulance services within the exclusive operating area located in Solano County. SEMSC, in its capacity as the previous LEMSA, executed the original agreement on April 8, 2010 for a contract term of May 1, 2010 through May 1, 2015. On October 3, 2014, a first amendment was executed to extend the term through May 1, 2020. A second amendment was executed on October 9, 2014, to provide that if certain conditions were met the contract would be extended for an additional five-years through May 1, 2025 and to authorize annual adjustments to Medic’s rates. On June 13, 2019, a third amendment was executed to exercise the five-year extension through May 1, 2025. A fourth amendment was executed on April 1, 2024, to increase the franchise fee through May 1, 2025. On May 1, 2025, the agreement was amended for a fifth time to extend the term through May 1, 2026, and to increase the franchise fee.
This sixth amendment for a six-month extension will allow for continuation of services during the completion of the procurement process.
FINANCIAL IMPACT:
The proposed amendment extends the revenue agreement with Medic from May 1, 2026, through December 1, 2026. Under this extension, the annual franchise fee will increase by $50,000, resulting in a total annual franchise fee of $719,000. This revenue will be prorated and billed monthly for a monthly franchise fee of $59,917, and a total franchise fee payable for the seven-month extension of $419,417.
ALTERNATIVES:
The Board of Supervisors may choose not to approve this sixth amendment. This is not recommended, as it may adversely affect essential advanced life support ambulance services within the exclusive operating area.
