DEPARTMENTAL RECOMMENDATION:
The County Administrator’s Office recommends that the Board of Supervisors approve a third contract amendment with Wipfli, LLC for $54,580 for a total contract of $344,589 to provide services related to the preparation of four Medi-Cal Change in Scope of Services requests and one Medi-Cal Home Office Report for the current period through June 30, 2027; and delegate authority to the County Administrator or designee to execute the third contract amendment.
SUMMARY/DISCUSSION:
The County entered into an agreement with WIPFLI LLC on January 6, 2025 to provide services related to the preparation of four Medi-Cal Change of Scope in Services requests and one Medi-Cal Home Office Report. The Health and Social Services, Family Health Services (FHS) adult clinics added chiropractic services and the pediatric clinic added behavioral health services and Wipfli, working closely with Health and Social Services, submitted the Change in Scope of Services requests to the California Department of Health Care Services (DHCS). These Change in Scope of Services and the Home Office Report are the mechanism by which the County applies for changes in its patient reimbursement rates. As part of its review process, DHCS assigns an audit team to review the requests and requires the County to provide any supplemental information it deems necessary in validate the request. This audit process has been extended twice with DHCS due to the volume of supplemental information requested. As a result, on June 24, 2025, and May 19, 2026, the County entered into first and second amendments to the original agreement to continue Wipfli’s support during the audit period. The requested third amendment extends Wipfli’s support through the end of the final, initial audit period. Once DHCS issues final rates for each submittal the County will determine whether to accept or appeal the rate decision.
FINANCIAL IMPACT:
The cost of the third amendment will be funded through the County Administrator’s Office FY2026/27 Working Budget. The costs associated with preparing the agenda item are nominal and absorbed by the Department’s FY2026/27 Working Budget.
ALTERNATIVES:
The Board could choose not to approve the third amendment; however, Wipfli’s expertise in responding to technical questions from DHCS is valuable in obtaining rate adjustments for the FHS clinics.
OTHER AGENCY INVOLVEMENT:
H&SS has been closely engaged, providing extensive data and supplemental information.
