DEPARTMENTAL RECOMMENDATION:

 

The Department of Health and Social Services (H&SS) recommends that the Board of Supervisors approve a fourth contract amendment with Canyon Manor for $80,812 for a total contract amount of $1,506,880 to cover costs incurred to provide sub-acute psychiatric residential treatment for seriously, persistently mentally ill persons with complex psychiatric needs in a locked Mental Health Rehabilitation Center for the period of July 1, 2023, through June 30, 2024; and delegate authority to the County Administrator to execute the amendment.

 

SUMMARY:

 

Canyon Manor is licensed as a 24-hour mental health rehabilitation center and locked treatment program that provides intensive rehabilitation treatment programming and nursing care for gravely disabled and severely mentally ill clients. In this setting, Canyon Manor supports the ongoing needs of sub-acute care clients with a personalized treatment plan, structured living environment, mental health education, regular evaluation of progress with the psychiatrist and an interdisciplinary treatment team, medication management, and residential care treatment. 

 

The contract with Canyon Manor was originally executed for $540,448 in July of 2022. In June 2023, a first amendment increased the budget by $705,620. In October 2023, a second amendment revised the budget detail to reflect updated rates. The third amendment increased the budget by another $180,000 in January 2024 for a total contract amount of $1,426,068. The services in this treatment setting cost $384.36 per client per day. Due to increased placement demand the cost for services will exceed that amount by an additional $80,812 before contract expiration. The requested increase reflects an increase in the daily rate and the placement of one (1) additional client, bringing the total to seven (7) beds for Solano County Behavioral Health clients for the remainder of the contract.  This increase will provide for approximately 210 additional bed days and is needed for the continued care of County clients. 

 

FINANCIAL IMPACT:

 

1991 Realignment will fund the $80,812 fourth amendment. Existing appropriations in the FY2024/25 Working Budget will cover this unanticipated expense.  There is no impact to the County General Fund.  The costs associated with preparing the agenda item are nominal and absorbed by the Department’s FY2024/25 Working Budget. 

 

DISCUSSION:

 

Locked mental health rehabilitation centers (MHRCs) and institutions for mental disorders (IMDs) are used when a client is psychiatrically evaluated to be chronically gravely disabled and placed under a mental health conservatorship by an order of the Court.  The purpose of mental health conservatorships is to provide individualized treatment, supervision, and living arrangements for people who have a severe mental illness while still protecting their rights.

 

Mental Health conservatorships, often referred to as LPS conservatorships because the Lanterman-Petris-Short Act governs them, can involve confinement in a locked psychiatric facility to alleviate the symptoms of grave disability, improve functional impairment, and increase independence through recovery-based treatment.

 

During the last year, Solano County Behavioral Health saw an increase in the complex psychiatric and specialized treatment needs of our clients, necessitating a higher level of care in locked settings. Obtaining an MHRC and IMD bed has become a significant challenge each year as overall demand increases generating a shortage for appropriate treatment beds. 

 

Canyon Manor, with its 40-year treatment history, has demonstrated the ability to provide this specialized level of care to meet the psychiatric and specialized treatment needs of the County’s most vulnerable behavioral health clients.

 

ALTERNATIVES:

 

The Board could choose not to approve this contract amendment. This is not recommended as this limits the available locked MHRC treatment programs for Behavioral Health clients with sub-acute and complex psychiatric needs and could result in having to move up to seven (7) clients currently receiving treatment for chronic disability.
