DEPARTMENTAL RECOMMENDATION:
The Department of Health and Social Services (H&SS) recommends that the Board of Supervisors approve a second contract amendment with St. Helena Hospital dba Adventist Health Vallejo for $806,771 for a total contract amount of $1,006,496 to provide additional acute psychiatric inpatient services for the current period through June 30, 2025; and delegate authority to the County Administrator to execute the amendment and any subsequent amendments, with County Counsel concurrence, up to an aggregate of $74,999.
SUMMARY:
St. Helena Hospital dba Adventist Health Vallejo provides acute psychiatric inpatient treatment services for Solano County Medi-Cal beneficiaries or indigent clients who are referred by Solano County Behavioral Health. Over the last few months there has been an increase in referrals/admissions to this facility. This second contract amendment will provide for the continuation of services that are recognized and accepted for the diagnosis and treatment of behavioral health disorders or psychological injuries.
FINANCIAL IMPACT:
This contract amendment will be funded by 1991 and 2011 Realignment. Appropriations are available in the FY2024/25 Working Budget. There is no additional impact to the County General Fund.
DISCUSSION:
Locked inpatient psychiatric hospitals are essential in providing a structured, therapeutic clinical environment to effectively and rapidly treat children, adolescents, adults, and older adults suffering from acute psychiatric issues. Clients receive an initial assessment, diagnosis, and rapid stabilization of acute psychiatric and co-occurring disorders (psychiatric and substance abuse disorders) with intensive physician involvement and oversight, a physician-led multidisciplinary treatment team, an individualized client treatment plan, therapeutic groups, educational classes, and comprehensive care coordination and discharge planning, including a smooth transition to less intensive, appropriate levels of follow-up care and support.
Obtaining an available inpatient psychiatric hospital bed remains a challenge as overall demand continues to increase and outpace the number of beds across the State. Over the last few months, inpatient psychiatric stabilization service utilization at this facility has seen a significant increase. In FY2023/24, admissions averaged one client per month with an average stay of 5 - 7 days. Current admissions are averaging eight clients per month with an average stay of 7 - 10 days per client. Additionally, the acuity level of clients has also been increasing. The higher utilization is primarily driven by placements from local emergency departments. This contract amendment will provide for approximately 488 additional bed days for these patients.
ALTERNATIVES:
The Board of Supervisors may choose not to approve the contract amendment. This is not recommended as it could pose a risk to individuals requiring inpatient psychiatric hospitalization.
