DEPARTMENTAL RECOMMENDATION:
The Department of Health and Social Services Behavioral Health Division recommends that the Board of Supervisors adopt a resolution redesignating David Grant Medical Center as a California Welfare and Institutions Code (WIC) Sections 5150 and 5404 involuntary detention facility for evaluation and treatment of adults who are experiencing acute psychiatric symptoms.
SUMMARY:
David Grant Medical Center (DGMC) at Travis Air Force Base in Fairfield, California, has been a designated involuntary detention facility for over a decade. The Board approved the last designation November 1, 2022 and redesignations are required every 3-years.
Clients admitted to DGMC may be held on a WIC Sections 5150 and 5404 hold, based on assessment at a local Emergency Department or the Crisis Stabilization Unit. As a locked psychiatric inpatient facility, DGMC must be designated a WIC Section 5150 and 5404 involuntary detention facility and authorized to perform evaluation and treatment for adults. The Department of Health and Social Services, Behavioral Health Division has worked closely with DGMC in meeting the requirements for WIC 5150 and 5404 designations. Additionally, Solano County Behavioral Health has contracted with DGMC’s psychiatric facility to ensure that patients’ rights, services, and information are shared with individuals under their care, including the right to contact the Advocate and the Office of Patients' Rights pursuant to WIC Sections 5520(d), 9 CCR 863.2(a)(1) and (a)(2).
FINANCIAL IMPACT:
Redesignating the David Grant Medical Center as a WIC Sections 5150 and 5404 facility does not create an additional impact on the Department's FY2025/26 Working Budget or the County General Fund.
DISCUSSION:
Involuntary Detention Facilities provide inpatient psychiatric care to some of Solano County’s most vulnerable individuals and provide stabilization and care coordination so these individuals can be discharged to available community support systems.
Every three years, Involuntary Detention Facilities operating within Solano County are subject to a review process conducted by the Behavioral Health Quality Assurance (QA) team before being recommended for redesignation as an Involuntary Detention Facility. According to the Department of Health Care Services (DHCS) Licensing and Certification Division, such designation is contingent on the following requirements:
• The County Behavioral Health Director needs to request approval for such designation from DHCS on behalf of the Board of Supervisors; and
• Attestation that indicates the program will meet or has the capacity to meet the requirements of Title 9, California Code of Regulations 663 "Minimum Staffing Requirements" and 821 "Approval of Facilities."
The Behavioral Health QA team has completed the evaluation and is recommending that the Board of Supervisors designate David Grant Medical Center as a Welfare and Institutions Code Sections 5150 and 5404 Involuntary Detention Facility. The Board’s designation will be submitted to California’s Department of Health Care Services (DHCS) along with the QA team’s site visit information and policy review before DHCS officially designates the facility for an additional three years.
ALTERNATIVES:
The Board may choose not to redesignate the facility as an Involuntary Detention Facility. This is not recommended because members of the Travis Air Force Base community, Solano County Medi-Cal and uninsured individuals requiring this level of care would significantly impact other Solano-designated facilities, creating fewer available beds countywide.
OTHER AGENCY INVOLVEMENT:
David Grant Medical Center on Travis Air Force Base collaborated and fully partnered with the Department of Health and Social Services Quality Assurance team to successfully complete the Involuntary Detention Facilities designation site and policy review.
