DEPARTMENTAL RECOMMENDATION:

 

The Department of Health and Social Services (H&SS) recommends that the Board of Supervisors approve a second contract amendment with Davis Guest Home for $513,775 for a total contract amount of $1,108,725 to provide intensive residential mental health services for the current period through June 30, 2025; and delegate authority to the County Administrator to execute the amendment and subsequent amendments, with County Counsel concurrence, up to an aggregate of $74,999.

 

SUMMARY:

 

On June 27, 2023, the Board of Supervisors approved a contract with Davis Guest Home for $594,950 for the period of July 1, 2023 through June 30, 2025 to provide intensive residential services for Solano County Behavioral Health clients. Davis Guest Home is an augmented board and care facility and provides intensive services which are an essential part of the behavioral health continuum. This second contract amendment is needed as the original contract with Davis Guest Home did not anticipate the need to provide a higher level of mental health services to higher acuity clients and a higher daily census average during the two-year agreement. The contract amendment is necessary to continue providing these services and concomitant housing for the individuals placed through the end of FY2024/25.

 

The services provided include daily mental health services, support for improved functioning, and individualized preparation in the skills necessary for the client to move to a board and care or other community-based housing arrangement.

 

 

FINANCIAL IMPACT:

 

Appropriations are available in the FY2024/25 Working Budget for this amendment. The contract is funded with 1991 Realignment and County General Fund. There is no additional impact to the County General Fund.

 

The cost associated with preparing this agenda item is nominal and absorbed by the Department’s FY2024/25 Working Budget.

 

DISCUSSION:

 

Davis Guest Home is an augmented board and care facility which provides intensive residential treatment for individuals who are chronically gravely disabled and who are often under Public Guardian conservatorship. Behavioral Health must provide for needed services to individuals conserved under Lanterman-Petris-Short (LPS) Act since they are not able to care for themselves and often require a higher level of structured residential care. These services include daily mental health services and support for improved functioning, and individualized treatment plans to aid in the development skills necessary to ameliorate the symptoms of grave disability with the goal of increasing independence, improving functioning, and assisting them to move to a lower level of care such as a board and care or other community-based housing arrangement.

 

Historically, H&SS Behavioral Health has been successful in identifying and transitioning clients from a higher service level locked mental health facility to a lower level of care such as this one. Davis Guest Home is currently averaging a census of seven clients at the daily rate of $165 for the moderate acuity client, the facility is also providing more intensive services to a client presenting with a higher acuity resulting in a greater per day cost of $350. Clients who are placed at Davis Guest Home have a more protracted history of mental health impairment and require longer periods of time to stabilize, decrease their grave disability, and reach their recovery goals that support a less structured environment.

 

The first amendment to the contract recognized adjustments in billing rates between fiscal years with no net increase in total contract amount.

 

ALTERNATIVES:

 

The Board may choose not to approve this contract amendment. This is not recommended as it would limit the available augmented board and care facilities for behavioral health clients with moderate to high acuity psychiatric needs and would result in having to move clients prematurely thus disrupting their treatment program prior to reaching their behavioral health treatment goals.
