Approve three contract amendments funded by Public Health: a first contract amendment with Persimmony International, Inc. from $236,748 to $318,460 effective March 28, 2017 through the current contract period of June 30, 2018 to expand the number of users by 16 for the electronic health record to include the Communicable Disease Program; a second contract amendment with SIMI Group from $255,000 to $385,500 effective March 28, 2017 through the current contract period of June 30, 2017 to develop an electronic tool in the community health care clinics to track and assess social determinants of health and other health data; and a second contract amendment with Full Court Press Communications, Inc. from $271,751 to $298,251 effective March 7, 2017 through the current contract period of June 30, 2017 to expand media outreach and advertising to promote chronic disease prevention and early intervention activities; and Authorize the County Administrator to execute the amendments and any subsequent amendments that remain within budgeted appropriations.

 

SUMMARY:

 

H&SS’s Solano Public Health is proposing three contract amendments as part of efforts to improve the health and well-being of Solano County residents. The three contract amendments include:

 

                     Persimmony International Inc. (Persimmony): the contract amendment will increase from $236,748 to $318,460 effective March 28, 2017 through the current contract period of June 30, 2018 to expand the electronic health record system to include the Communicable Disease Program and the number of users of the electronic health record from 29 to 45 employees.

                     SIMI Group: the contract amendment will increase from $255,000 to $385,500 effective March 28, 2017 through the current contract period of June 30, 2017 to develop an electronic tool in the community health care clinics to track and assess social determinants of health and other health data. These data will be provided to the health care provider to inform treatment of care for the client.

                     Full Court Press Communications, Inc.: the contract amendment will increase from $271,751 to $298,251 effective March 7, 2017 through the current contract period of June 30, 2017 to expand media outreach and advertising to promote chronic disease prevention and early intervention activities. The increase in funding will secure additional media buys and additional materials and signage for healthy food and beverage purchases at partner convenience store locations.

 

FINANCIAL IMPACT:

 

The total funding of $238,712 for the proposed contract amendments is from a combination of funding sources, including Centers for Disease Control and Prevention ($26,500), Public Health Realignment ($177,212), and a grant from County Medical Services Program ($35,000). There are no County General funds associated with these contracts. 

 

DISCUSSION:

 

These contract amendments support H&SS’s Solano Public Health’s vision of Healthy People-Healthy Community. Each of these contractors has had long-standing relationships with Solano Public Health and collaborative community efforts to improve health outcomes for Solano residents. The three contract amendments proposed include:

 

                     Persimmony International Inc. (Persimmony): since July 1, 2015, H&SS’s Solano Public Health has worked with Persimmony to establish an electronic health record system for District Public Health Nurses. This contract amendment will expand the electronic health record system to the Communicable Disease Program in Solano Public Health. The number of users for the electronic health record system will increase from 29 users to 45. By moving the Communicable Disease Program to an electronic health record system the program will improve efficiencies by entering the data into an electronic system; exporting data into a state-wide reporting system; and improving capabilities to analyze communicable disease data for Solano County. Public Health Realignment funds will support this expansion.

                     SIMI Group: since Fiscal Year 2013/14, H&SS’s Solano Public Health has worked with the SIMI Group to develop a Public Health Hub. The Public Health Hub collects health data from various health care providers, including local hospitals, physicians, and community health centers in one location to analyze and assess the health status of Solano County residents. This contract amendment will expand services to include a cloud-based analytic tool to collect and analyze social determinants of health and other health indicator data in local community health centers. County Medical Services Program recently awarded Solano County a grant to implement this activity.

                     Full Court Press Communications, Inc.: since Fiscal Year 2015/16, H&SS’s Solano Public Health has contracted with Full Court Press Communications, Inc. to serve as the lead media provider for advertising and media outreach related to improving access to healthy fruits, beverages, tobacco-free environments and opportunities for chronic disease prevention and management. In 2015, Solano Public Health received a grant from Centers for Disease Control and Prevention for the Partnership to Improve Community Health (PICH), and as part of this scope of work with Centers for Disease Control and Prevention, Solano Public Health shall implement a communications outreach strategy to address chronic disease prevention and early intervention. The amendment also provides materials to convenience stores who are partnering with Solano Public Health to promote healthy food and beverage purchases at these locations. This contract amendment expands media outreach placement in Fiscal Year 2016/17.

 

All contracts and contract amendments support H&SS’s Solano Public Health efforts to improve the quality of life for Solano County residents where they live, work, and play.

 

ALTERNATIVES:

 

The Board may choose not to approve the contract amendments. This is not recommended because it would prevent H&SS’s Solano Public Health from expanding its electronic health record to the Communicable Disease Program; establishing an electronic tool in community health centers to track and analyze social determinants of health; and expanding media outreach to promote the prevention and early intervention of chronic diseases.

 

OTHER AGENCY INVOLVEMENT:

 

H&SS worked with the County Administrator’s Office and County Counsel to develop the contract amendments. Also, H&SS’s Solano Public Health worked with the Community Clinic Consortium, and the contractors Persimmony International, Inc. and SIMI Group.
