DEPARTMENTAL RECOMMENDATION:

 

Solano County Health & Social Services, Solano Public Health recommends the Board of Supervisors receive a presentation and report about the 2017 Community Health Improvement Plan for Solano County.

 

SUMMARY:

 

On March 14, 2017, the Board received a presentation on the Community Health Needs Assessment. The objectives of the Community Health Needs Assessment were to identify community health issues within Solano County and identify resources available to address those issues. The data from the Community Health Needs Assessment were used to develop the 2017 Community Health Improvement Plan (Attachment A) for Solano County.  In addition to representing important information about the health needs of the community and the strategic priorities identified to address those needs, the Community Health Needs Assessment and 2017 Community Health Improvement Plan, respectively, are necessary elements for Solano Public Health to achieve Public Health Accreditation by the Public Health Accreditation Board.

 

Solano Public Health convened the Healthy Solano Collaborative (made up of a diverse group of departments, agencies and organizations that represent the local public health system) to develop the 2017 Community Health Improvement Plan last year. The 2017 Community Health Improvement Plan for Solano County addresses strategic health issues, including: poverty; homelessness; lack of affordable housing; inequitable K-12 education and barriers to educational attainment; and unemployment.

 

FINANCIAL IMPACT:

 

The Health & Social Services staff time associated with these events is budgeted within the Department's Fiscal Year 2017/18 budget. There is no impact to the County General Fund.

 

DISCUSSION:

 

A Community Health Needs Assessment for Solano County was completed in 2016 and presented to the Board on March 14, 2017 by the Solano Community Health Needs Assessment Collaborative, a collaborative of the three nonprofit hospital systems-Kaiser Permanente, NorthBay Healthcare and Sutter Health Sacramento Sierra Region, as well as Solano Public Health and the Solano Coalition for Better Health. Based on data in the Community Health Needs Assessment, health issues that impact the health and wellbeing of Solano County residents were identified and prioritized to determine which strategic health issues would be addressed in the 2017 Community Health Improvement Plan for Solano County.

 

The purpose of the Community Health Improvement Plan is to provide guidance to the local public health system, its partners, and stakeholders for improving the health of the population. It is a document capturing the priorities and commitment of Solano County residents and of agencies and organizations in the County to create healthier communities for everyone. The strategic health issues identified in the 2017 Community Health Improvement Plan include:

 

                     Poverty,

                     Homelessness & Lack of Affordable Housing,

                     Inequitable K-12 Education & Barriers to Educational Attainment, and

                     Unemployment

 

Solano Public Health convened the Healthy Solano Collaborative to identify and prioritize the strategic health issues to be addressed in the 2017 Community Health Improvement Plan. The Healthy Solano Collaborative is a diverse group of departments, agencies and organizations that represent the local public health system and play a role in impacting the health of the public. Once the strategic health issues were identified, the Healthy Solano Collaborative developed initial implementation plans, including goals, objectives and first steps to begin addressing these issues.

 

A key element of the process for developing the 2017 Community Health Improvement Plan has been developing partnerships and capitalizing on the great work already being done in our diverse community.  The strategic health issues identified in the Plan represent large and significant problems facing most communities, and it is not anticipated that these issues can be successfully addressed by the resources of Solano Public Health or even Solano County alone.  Instead, it is expected that the resources, activities and initiatives of existing entities focused on these strategic health issues will be leveraged and coordinated with the assistance of Solano Public Health and the Healthy Solano Collaborative to help achieve meaningful successes.

 

For each initial implementation plan, partnerships have been formed with entities that are already working on the strategic issue. For example, Solano Kids Thrive - a collective impact group of agencies serving adolescents in Solano County - has been working towards raising awareness about adverse childhood experiences (ACEs) and building resilient communities in Solano County. Addressing ACEs is one of the goals for the Community Health Improvement Plan strategic issue regarding inequitable K-12 education and barriers to educational attainment. By working with and supporting Solano Kids Thrive’s mission and strategic plan, the Healthy Solano Collaborative avoids duplication of services while augmenting Solano Kids Thrive’s work. This is just one example of the partnerships that have been formed with existing organizations working on the strategic health issue areas identified in the 2017 Community Health Improvement Plan.

 

Solano Public Health will periodically convene the Healthy Solano Collaborative to monitor progress with implementation of the 2017 Community Health Improvement Plan and discuss gaps in interventions, alignment of efforts and future collaborations. The implementation plans for each strategic health issue identified in the 2017 Community Health Improvement Plan include metrics for indicating success, which will be tracked and evaluated on a quarterly basis at the Healthy Solano Collaborative Meetings.

 

Solano Public Health included $200,000 in the Fiscal Year 2017/18 budget to support the initial implementation plans for the strategic health issues identified in the 2017 Community Health Improvement Plan. It is anticipated that these one-time funds will be used to support each of the four health issue specific workgroups in developing strategies and longer term implementation plans to address the health issue. A request for proposals will be issued in early 2018.

 

With the completion of the 2017 Community Health Improvement Plan, Solano Public Health will move to the next phase of Public Health Accreditation. Solano Public Health projects completion of the Public Health Accreditation process by summer of 2019.

 

ALTERNATIVES:

 

The Board may choose not to receive the presentation on the 2017 Community Health Improvement Plan for Solano County. This is not recommended because this presentation supports efforts to improve the health and well-being of those who live, learn, work, and play in Solano County.
