DEPARTMENTAL RECOMMENDATION:
The Sheriff recommends that the Board of Supervisors:
1. Affirm the submission of the California Department of Health Care Services (DHCS) - Providing Access and Transforming Health Justice-Involved (PATH-JI) Reentry Initiative Capacity Building Program Grant - Round 4 application and expenditure plan;
2. Affirm the acceptance of DHCS’ Round 4 award of $1,000,000 to support PATH-JI activities;
3. Affirm the acceptance of $440,294 in unexpended Round 1 and Round 2 PATH-JI funds redistributed by DHCS and the Sheriff’s modified Round 3 expenditure plan;
4. Delegate authority to the Sheriff to execute all necessary documents associated with PATH-JI Round 4 activities and any future amendments, pending County Counsel concurrence;
5. Adopt a resolution amending the List of Numbers and Classification of Positions to add 1.0 FTE Accounting Clerk II in the Inmate Programming Division of the Sheriff’s Office and extend the Limited-Term Project Manager through June 30, 2028;
6. Approve a first contract amendment for $300,000 with Health Management Associates, Inc. for a total contract amount of $639,986 to provide project management, consulting, and technical assistance services related to CalAIM through November 30, 2027;
7. Approve a second contract amendment for $227,540 with HealthRight 360 for a total contract amount of $1,737,060 to provide in-custody case management, re-entry services, and implementation of CalAIM justice-involved initiatives through June 30, 2027;
8. Approve a first contract amendment for $118,704 with the Society of St. Vincent de Paul Sacramento for a total contract amount of $1,785,455 to provide mental health jail re-entry services and implementation of CalAIM justice-involved initiatives through June 30, 2027; and
9. Approve an Appropriation Transfer Request for $276,150 recognizing unanticipated grant revenue offset by grant related appropriations (4/5 vote required).
SUMMARY/DISCUSSION:
Under the California Advancing and Innovating Medi-Cal (CalAIM), correctional facilities were originally required to go-live with 90-day Pre-Release Medi-Cal services by April 1, 2026, which was then extended to October 1, 2026. However, due to statewide implementation issues experienced by some counties, correctional facilities are now required to go-live October 1, 2026, with operational re-entry services, Medication-Assisted Treatment services, and pharmacy release medication services. The Sheriff’s Office has met these requirements and expects to receive a “Conditional” letter that grants Solano County permission to go-live by the October 1 deadline. The remaining requirements must be implemented within one year of the receipt of the “Conditional” letter.
On March 12, 2025, the California Department of Health Care Services (DHCS) released a memorandum with guidance related to PATH-JI Capacity Building Program Round 4 funding. Round 4) provides additional funding to support the planning and implementation of targeted pre-release Medi-Cal services for individuals in state prisons, county jails, and youth correctional facilities. The Sheriff’s Office applied for funding through this initiative on August 25, 2025, and was recently awarded $1,000,000. Path-JI Round 4 funding will augment Round 3 funding to support the implementation of processes necessary to provide targeted pre-release Medi-Cal services to individuals currently in our jail facilities and to ensure continuation of care upon release.
The Sheriff’s Office will continue in its efforts to comply with the CalAIM mandate. The requested contract extensions with Health Management Associates, Inc. (providing consultant services), Society of St. Vincent De Paul Sacramento (providing inmates with mental health issues case management and re-entry services), and HealthRight 360 (providing inmate case management and re-entry services) are necessary to continue providing supplemental CalAIM services. Additionally, the extension of the Limited-Term Project Manager and the addition of 1.0 FTE Accounting Clerk II, with the intent to fill at the Account Clerk I level, are critical to project success as these positions will assist in operationalizing a Medi-Cal billing system for the Sheriff’s Office.
FINANCIAL IMPACT:
Board action will have no impact to the County General Fund. The original PATH-JI Round 3 grant award was $3,500,000 and, together with redistributed unexpended Solano County PATH-JI Round 1 funds ($87,129.02) and PATH-JI Round 2 funds ($244,175.62 Sheriff; $108,989.15 H&SS), Round 3 funding now totals $3,940,294.
The DHCS has approved the Sheriff’s budget modification for Round 3 (Attachment A). The estimated FY2026/27 cost is $2,405,224 as the Sheriff’s Office anticipates continuing costs for contracted consultant services; information technology services from DoIT and third-party IT providers under Master Agreements with DoIT; contracted case management and re-entry services; the Limited-Term Project Manager; and the Accounting Clerk II position. The annualized cost for the Accounting Clerk II position, which is intended to be filled at the Account Clerk I level, is $123,144; however, the estimated FY2026/27 cost is $71,834 as the position is anticipated to be filled on December 1, 2026. The PATH-JI Round 3 budget has sufficient appropriations.
The PATH-JI Round 4 grant award is $1,000,000 and is delineated in the program budget (Attachment B). The Sheriff’s Office has already received the full award in advance funding. The estimated FY2026/27 cost is $276,150 to continue to fund contracted consultant services, to purchase office furniture for carrying out CalAIM activities, and to purchase needed supplies to issue to inmates upon release. The PATH-JI Round 4 budget does not have sufficient appropriations; therefore, an Appropriations Transfer Request of $276,150 is necessary to increase appropriations and recognize unanticipated offsetting grant revenue.
The Sheriff’s Office will include PATH-JI FY2027/28 program costs in its Recommended FY2027/28 budget. These ongoing costs shall be fully offset by PATH-JI Round 3 and Round 4 funds.
The costs associated with preparing item are nominal and absorbed by the department’s FY2026/27 Working Budget.
ALTERNATIVES:
The Board may choose not to approve one or more of the contract amendments and/or not to extend the Project Manager (Limited-term) position or approve the resolution to add the Accounting Clerk II position; however, these alternatives are not recommended because without the services provided by the contractors and Sheriff staff, the Sheriff’s Office will be unable to meet the CalAIM PATH-JI implementation and readiness mandates of the state.
OTHER AGENCY INVOLVEMENT:
County Counsel has reviewed and approved the contract amendments as to form. Human Resources has reviewed and approved the Accounting Clerk II classification and has prepared the resolution to amend the List of Numbers and Classification of Positions.
